Healthy Communities Coalition

Formed in 2010, the Healthy Communities Coalition resulted from a consolidation of many health coalitions (Asthma, Physical Activity, Champions, and Cancer). The Coalition’s membership consists of local government officials, public health, hospital, Fire/EMS, Chamber of Commerce/Industrial Authority, MCO’s, Cooperative Extension, and local business representatives.

In January 2013, workgroups were formed around each of the identified strategic issues from the Community Health Assessment to work toward the development of a work plan to improve the health of our community in each of these areas. The Communication workgroup formed in 2016. The MAPP Partnership also identified the need for, and adopted an organized structure to, oversee and guide the community health improvement project, illustrated below.

Nearly every member of this Coalition was involved in this project which made the Coalition the most logical entity to provide oversight for the health improvement work.

Over the course of time between the 2013 assessment and the 2016 assessment, each workgroup re-formulated goals and strategies to address their respective strategic issue. These are compiled into the 2016 Montgomery County Community Health Improvement Plan.
